First Visit
Your initial consultation is a crucial step in the treatment planning process. Our office prides itself in detailed pre-operative consultations so that we can deliver the very best clinical care and anticipate surgical outcomes.
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Important information we will gather at this time includes your medical history, a brief physical exam, the reason for your visit and any radiographs, if necessary. The doctors will then review this information and recommend a treatment. They will also explain the treatment in detail, because understanding the treatment is important in making you comfortable with your procedure. Dr. Clark and Dr. Warren have many years of clinical and surgical experience, which enables them to answer any questions you might have.
 
Your consultation visit will conclude with a brief visit with one of our financial coordinators. They will go over the proposed treatment plan and the associated fees and find a convenient time to schedule your surgery.
 
We know your time is valuable, so in order to streamline this process, you may do several things prior to and the day of your consultation appointment:
 
  Your personal and insurance information and a brief health history can be filled out prior to your visit. Simply click on ‘Patient Registration’ button on our website home page and fill out the form. You are not required to sign or print the form. You need only to click the SUBMIT button at the bottom.  
  If you have dental or medical insurance you will be using for your visit and treatment, please bring your cards with you so we may make copies of them.  
  In addition to your insurance cards, we will also make a copy of your photo identification (i.e. driver’s license, student ID).  
  Please alert the clinical staff if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.). Also, if you take medications, please bring a list of these with you.  
  If you have a medical and/or dental HMO policy, you will also need to provide an authorization from your insurance company. If we will be filing insurance for you, please refer to our insurance section for further instructions.  
  Finally, be prompt for your appointment time. We have a great deal of information to cover and the exact amount of time necessary has been allotted especially for you. You should expect your consultation to take approximately one hour.
